Please note that once you establish care with a provider at one of our clinics, that provider will remain your primary provider for ongoing care. This approach supports continuity of care, ensures familiarity with your health history, and helps us deliver the best possible care experience. As a result, continuing treatment may require visits at the same clinic location where you initially established care.

Our dental department currently only serves residents of Crawford, Grant, La Crosse, Monroe, Sauk, and Vernon counties in Wisconsin. If you live in a county outside our service area, you do not qualify to register for a dental appointment at this time
