Dental Appointment Request Form

Please complete a registration form for each household member interested in a dental exam. Our dental department serves patients from Monroe, Vernon, Crawford, Grant and La Crosse counties in Wisconsin.

Our staff are using this form to contact patients as appointments are available throughout the month. At the end of the month, if you have not been contacted to schedule an appointment, you can register again during the next open scheduling period of the following month.

Frequently Asked Questions

Why are we asking you to register?

In the last year, Scenic Bluffs has experienced limited access for dental patients due to a shortage of available dentists. This shortage made it difficult to meet the large demand we have for dental patient appointments.  Scenic Bluffs is happy to have a growing team of dental professionals, making it possible to schedule daily patient exam appointments for new or existing patients. However, we are still managing a large demand for services.

 

How is this form used?

Our staff are using this form to contact patients as appointments are available throughout the month. After the completion of an exam appointment, all patients can continue scheduling care with our dental team to complete their treatment plans.

At the end of the month, if you have not been contacted to schedule an exam appointment and remain interested in dental care at Scenic Bluffs, you can register again during the next open scheduling period of the following month.  

 

Who do I contact for assistance using this form?

Patients that are unable to use this online registration can visit in-person or call Scenic Bluffs during the open scheduling period of the month for registration assistance. Please know that our staff do not give any scheduling preference based on how a patient registers into this process.

 

Will there be more appointment available in the future?

As we continue to recruit and grow our dental team and as we bring on new dentists, the number of available exams will increase.

 

Why don't I see my dental insurance listed on the request form?

If you do not see your dental insurance company listed as an option in the registration form, it is likely that we do not have a dentist currently available for exams that is enrolled with that insurance.  This means that your insurance will not pay for services from our dentists. This list changes often and we encourage you to check back in the future.

 

Are pain appointments still available?

If you are in pain, please call Scenic Bluffs at 608-654-5100 to get information about our standby appointments.

 

I don't have dental insurance. Am I able to request an appointment?

Yes, please fill out the dental request form. Scenic Bluffs cares for those without insurance and no one is denied services for lack of insurance. We also encourage all patients to fill out our sliding-fee scale application at their first appointment, called our Healthy Neighbor Plan.
