Winning Gold+ in Target BP Program

Winning Gold+ in Target BP Program

For the second consecutive year, Scenic Bluffs has received the prestigious Gold+ Award from the American Heart Association (AHA) for participation in the Target BP program. This recognition highlights commitment to improving heart health through innovative programs, such as the Self-Monitoring Blood Pressure (SMBP) initiative, which launched in spring 2023.

The SMBP program empowers patients to take control of their heart health by regularly measuring their blood pressure at home. By using a blood pressure monitor outside the doctor’s office, patients can get more accurate readings in familiar, stress-free environments. This allows healthcare providers to better assess blood pressure trends over time, ensuring that patients receive more personalized and effective care. Patients participating in the SMBP program are provided with a take-home kit that includes a blood pressure cuff, instructions, educational materials, a calendar/log, and a pen. We also offer hands-on tutorials to help patients properly use the equipment and track their readings, making it easier to work with their care team to develop a treatment plan.

In late 2024, Scenic Bluffs received a $1,000 grant from Walmart Distribution Center in Tomah to purchase extra blood pressure cuffs for patients, ensuring that more community members have access to this life-saving program. We grateful to everyone who has supported this initiative and contributed to Scenic Bluffs’ success in receiving the Gold+ Award! Together, we are making a meaningful impact on heart health in our communities.
