How do I get access?

Patients must regsiter for a Patient Portal account by filling out the form below. Once you have completed the form, Scenic Bluffs staff will create your account. Please allow up to 48 hours for set up. You will then receive a welcome email with the link and instructions needed to finish setting up your account.

How do I request access for another patient?

0-11 years old:

If the patient is 0 to 11 years old, the proxy user must fill out sections 1 and 2 and sign section 4 of the Portal Access Form.

If the patient is 12 years or older, please fill out sections 1 and 2 of the Portal Access Form.

  • The patient must sign section 3 requesting access to their own portal account and section 5 granting proxy user access via a proxy user account. The proxy user must sign section 5 requesting a proxy user account.

Patient Portal Password Guidelines

Password must be at least 8 characters and include:

  • Uppercase and lowercase letters
  • A symbol
  • A number

DO NOT

  • Use words that can be found in the dictionary
  • Use a password that contains personal information
  • Use keyboard patterns (qwerty)
  • Use sequential numbers (1234)
  • Make your password all numbers, uppercase or lowercase letters
  • Use repeating characters (aaazzz)

If these guidelines are not followed and you click the NEXT button, you will not be able to log in. You will stay on the same screen until you create a password that follows the above password guidelines.
