Please arrive 15 minutes early to your first appointment so we can ensure all of your information is complete and correct. We ask that behavioral health patients arrive 30 minutes early to their first appointment.
What to bring for your appointment
Please bring the following information to your first appointment:
Completed required forms (can be found on the Forms page)
A list of your current medications
*If you do not have health insurance, are unemployed, or have an income level that makes it difficult for you to pay for health care, we offer a sliding fee scaled called the Healthy Neighbor Plan.
You are encouraged to bring a copy of your living will / advance directive (if you have one).
You may also consider creating a list of questions in advance to ask your medical provider.
To care for the many patients needing services, and to assure our clinicians can give you the professional time and attention you need, it is our policy to give preference to the patients who keep their appointments and show up on time.
Patients more than ten minutes late for an appointment or who do not show up at all will be counted as a “no show”. Late patients are given “walk-in” priority, but the “no show” is recorded on your account. When you “no show” three times within 12 months, you may be considered for termination from the practice.
Patients will be seen in the following priority: emergencies, sick children, appointed patients, urgent walk in patients, non-urgent walk in patients, then standby patients.
If you are aware of a conflict with your appointment time, call us 24-hours in advance to reschedule.
Parking is available in front of both our Cashton and Norwalk buildings. Handicap parking is available.

Scenic Bluffs is proud to offer a Help Team to guide you through locating, understanding, and obtaining healthcare benefits. Our staff is knowledgeable about the healthcare policy changes, insurance, and prescription drug programs. Our team can screen for potential eligibility in various programs and assist in submitting an online application for the Marketplace Insurance Exchange and other programs.
